Paizo and the artist himself seem to prefer Kuba Witowski over Jakub Adam Witowski. His website, dA, blog, Twitter, conceptart.org profile, recent Pathfinder Tales art credits, and email address all use Kuba Witowski. The only credits I've seen that use his full name are a single Paizo Blog post from March and a RPGGeek listing; another Paizo Blog post credits him as Jakub Witowski. --Oznogon (talk) 00:04, 29 January 2015 (UTC)

It seems like "Kuba" is the artist's preferred name, and that Paizo's use of other names was an error. I think it makes the most sense to use the name that appears most often, since it's the one people are likely to Google or that will, in theory, be used going forward.—Paizo Publishing, LLC.png Yoda8myhead (talk) 00:26, 29 January 2015 (UTC)
If there's no opposition, then on Monday (Feb. 2) I'll switch all of his artwork credits to Kuba Witowski, move the category contents over, and delete this category page. --Oznogon (talk)
Hmm, interesting question. We already have Mike Brock, Liz Courts, Rob McCreary, etc., so loads of precedent for this. Also Wikipedia seems to use shortened names too for page titles: see Wikipedia:Article titles for their policy. We should, though, have 'Jakub Adam Witowski' as a redirect to 'Kuba Witowski' and as a variant name on the page. See Bill Clinton for an example.
